• You must select a medical plan by Monday, December 3 to receive medical benefits for 2013. You also must choose a Primary Care Provider (PCP) for yourself and each family member. If you don’t enroll online by December 3, you will not be covered for medical benefits in 2013.
• Your other benefits – dental, vision, life and long-term disability insurance – continue in 2013 if you don’t make changes.
• You also must enroll in a 2013 Flexible Spending Account if you want to participate.

Why I Give
I have decided to donate money to Allen Foundation in the past for many reasons. First, there was the new Heart Center building. I know how important this addition was to our hospital for its growth and development. As an employee, I felt responsible for helping in any way I could. Improvements like this have a huge impact on the services we can provide to our community. You never know when you or a loved one will end up in the ER or in for a heart procedure. I have been there myself and with family and have enjoyed the improvements greatly. |

Secondly, I have chosen to donate to Allen College Class Scholarship as part of the “matching gifts” incentive. Being a nurse for 20 years and seeing many students come through our unit for clinical has helped me realize that we are all responsible for the development of our new nurses. They need our help to get the education they need to in turn help us in the future! As a member of the Allen Alumni Board, I have seen the need from a different angle and have come to realize that donating money toward scholarships also helps our College.
I believe it is important to help in a cause you believe in any way you can. The amount isn’t what’s important, it is the choice to give that is.

Allen Credit Union Information
New Loan Information:
Vehicles 2 years old or newer and under 30,000 miles may now be financed up to 84 months. Extended warranties are available on new purchases and currently owned vehicles with no waiting period. Call or stop in the Credit Union for a quote.
Holiday Hours:
The credit union will be closed Monday, Dec 24 and Tuesday, Dec. 25.
Holiday Open House:
Friday, December 13 7:00am to 4:00pm. Refreshments and prize drawing.
Skip-A-Payment Program:
There's still time to keep a little extra money in your pocket for the holiday season. Stop in for more information or to fill out an application. For a $20 fee, you may skip one monthly payment (or two bi-weekly payments) on any loan to help take some of the stress off of all the extra holiday expenses.

West High School Pawsitive Pet Group Students “BEG” For Your Help!
To support the local Cedar Bend Humane Society, these animal-loving, caring, and compassionate students are asking fellow animal-loving Allen employees to donate much needed supplies. Any donations may be taken to the Allen contact person, Peggy Short, in the Wound Center. For larger quantity donations pick-up is available by calling 815-499-0115 or extension 5195. Needed supplies: bleach, cat litter, Simple Green, laundry detergent, blankets (new or used), dish soap, Latex & rubber gloves, pet food (any brand), paper towels, toilet paper, scrubbing brushes, rubbing alcohol, peroxide, towels (new or used), toys & treats for dogs/cats, Q-Tips, leads & collars, heating pads, gauze, Clorox wipes, and stainless steel food bowls.
